Your privacy, our priority
At Elevate Life Care Co, we respect your privacy and are committed to protecting your personal information. We handle all information in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs).

Information we collect and why
To provide safe and effective support services, Elevate Life Care Co collects specific personal information. This includes your name, address, phone number, email, date of birth, gender, and relevant health or medical details. For NDIS participants, we also collect your NDIS number and plan specifics. Emergency contact information, support needs, and goals are also gathered to tailor our services. For job applicants, employment information is collected.
We only collect what's necessary to deliver services and meet legal obligations. We primarily collect this directly from you when you contact us, complete forms, or provide NDIS documents. With your consent, we may also collect information from nominated family members, carers, or support coordinators.

How we use and share your information
Your information allows us to deliver personalised disability support, develop and review your support plan, and communicate effectively with you and your support team. It's crucial for meeting our NDIS obligations, managing administration, billing, and processing employment applications.
We will not use or share your information for unrelated purposes without your explicit consent, unless required by law. We may share necessary information with our trained staff and health professionals involved in your care, your nominated representatives, support coordinators, and government or regulatory bodies when legally required. We ensure only essential information is shared with appropriate safeguards.

Protection and your privacy rights
Elevate Life Care Co takes robust steps to protect your personal information from unauthorised access, loss, misuse, or disclosure. Our measures include secure digital systems and strict access controls for sensitive records.
As an NDIS provider, we adhere to NDIS Practice Standards and Quality and Safeguards Commission requirements, ensuring guidelines around privacy, dignity, and confidentiality are met. You have the right to request access to your personal information and seek corrections if needed. Please contact us in writing, and we will respond within a reasonable timeframe.
